Health Education England London and Kent, Surrey, Sussex (LKSS) are pleased to provide details of the application process for 2023-24 which will allow applications for New Educational Supervisor Sites for Dental Foundation Training.

Please read this in conjunction with the 2023-24 guidance document (that includes the ES person and site specification) attached where you will be able to assess your eligibility for recruitment and find more detailed guidance around the process for this year. Key dates for Dental Foundation Recruitment of new sites are below:

LDC

The process requires you to complete: –

  1. Online Microsoft team form for eligibility /application: https://forms.office.com/e/HfWiNNgCDh
  2. Online Microsoft team form for practice descriptor: https://forms.office.com/e/VW3Be45YLR
  3. This will be the information prospective trainees use to preference your practice so please include all relevant details. We will not chase for information after this deadline.
  4. Email your supporting document (Activity Actual 22/23 for the training practice. PLEASE NOTE THAT THIS DATA MUST RELATE TO THE CONTRACT TO WHICH YOU PROPOSE TO ADD THE FD TO) to dft.lase@hee.nhs.uk at the time of your application, with clear title of email stating “EVIDENCE FOR NEW DFT SITE RECRUITMENT”.
  5. This will be the information prospective trainees use to preference your practice so please include all relevant details. We will not chase for information after this deadline.

Please note that appointment of new sites for DFT will be prioritised for areas of NHS need. These currently include Surrey and Southwest London.

Please can you also observe the following:

  1. The new trainer recruitment process is being managed by the Dental HET team and all queries relating to the application process should be directed to dft.lase@hee.nhs.uk. We ask that you DO NOT contact TPDs, ADs or other dental teams directly and that all correspondence comes to this mailbox.  Please be assured that this mailbox will be monitored on a daily basis throughout the process.
  2. That you submit one application form per trainer, with joint applicants submitting separate forms.
  3. For practice descriptor form only one per site.
  4. Please ensure that your application forms are fully checked and completed with no further need to change before submitting. Your final application form will be taken as your application and no further changes can be made.
  5. Please ensure that you complete the application form, the practice descriptor form AND email the activity profile evidence for your practice.
  6. Please note the deadline for submission is 9.00 am on the 28th February 2023. The on-line application form, practice descriptor form will not be available beyond this date and time, so we ask that you carefully consider the timeline during this process. Late applications and/or document submissions will NOT be considered under any circumstances.
  7. Please note you will NOT receive a confirmation email upon completion of the application forms so we recommend that you save copies of your forms for your own records.
